Biographical profile for use in the final round program
Hi-res photo (preferably 800 x 1000 pixels, full color)

After the application has been submitted, you will receive a confirmation email with the data you submitted. If you find errors, please write to juniorstarsofssba@gmail.com so we can amend your application.

Only a legal adult, age 18 or more, may complete this form.

I give Steinway Society The Bay Area (SSBA) permission to distribute, broadcast, and publish the video recording(s) in their original form on all platforms, including but not limited to steinwaysociety.com, YouTube, and Facebook. I also give SSBA full permission to share screenshots, clips, or other portions of my video recording(s) for promotional purposes.

To participate in the Junior Stars of SSBA, I understand that all the information on this application form is true. I understand that incomplete applications (including failure to pay fees and failure to fill in the form properly) will result in disqualification, and that all decisions made by the panel of judges are final and may not be appealed. I hereby certify that I have read and understood the Rules and Regulations of the Junior Stars of SSBA.
